The phrase is everywhere and the physics behind it is quoted wrongly almost every time. What is real is that your neck is holding one position for eleven hours a day.

The number you have seen, and what is wrong with it

You have probably read that looking down at a phone puts sixty pounds through your neck. That figure comes from a computational model, not from a measurement in a living person, and it is regularly presented as though somebody weighed it.

The direction of the finding is not controversial — flexing the head forward does increase load on the cervical structures, and the increase is substantial. What is oversold is the precision and the implied consequence, and overselling it has made the whole idea easy to dismiss.

What we actually see

Not damage from phones. What we see is people holding a single sustained posture for most of their waking hours, with the deep neck flexors doing very little and the upper trapezius, levator scapulae and suboccipital muscles doing continuous low-level work they were not designed for.

The result is myofascial pain, headaches referred from the upper cervical segments, and pain between the shoulder blades. It is a load-exposure problem rather than an injury, which is why it responds to changing the exposure.

The real driver is duration, not angle

The most useful reframe available: a neck can tolerate almost any position briefly and struggles with any position held for hours. The people who get this are not the ones with the worst posture — they are the ones who do not move.

Which means the intervention that works is not a perfect ergonomic setup. It is interruption. A neck that changes position every twenty or thirty minutes tolerates a mediocre workstation far better than a rigid neck tolerates an excellent one.

What to change, in order of how much it matters

  • Move more often. Every twenty to thirty minutes, briefly. This outranks everything else on the list.
  • Raise the screen to roughly eye height so the neck is not sustained in flexion.
  • Support the arms. Unsupported arms load the shoulder girdle and the neck continuously.
  • Bring the phone up rather than the head down, which is the only genuinely phone-specific item here.
  • Strengthen deliberately. Deep neck flexors and the scapular stabilizers, progressively loaded. Stretching alone gives relief that lasts hours.

When it is not just posture

If the pain travels past the elbow, if there is numbness in a defined strip, or if there is genuine weakness, that is a nerve root question and not a workstation question. If a headache is side-locked and starts at the base of the skull, look at cervicogenic headache.

And if the pain no longer settles on the weekend, the pattern has changed. Pain that used to resolve on Saturday and no longer does is the single most useful thing a person in this group can notice about themselves.

Whether posture causes pain at all

Stated honestly: the relationship between measured posture and neck pain is weaker than the popular version suggests, and people with visibly poor posture and no pain are common. What holds up better is the relationship with sustained static loading and with how conditioned the supporting muscles are.

That is a more useful model anyway, because it points at things that can be changed. Nobody has ever fixed a neck by achieving a perfect head position and then holding it for nine hours.

What to do about the headaches

Screen-related headaches are usually a mix of two things: referral from the upper cervical segments and referral from the muscles working continuously to hold the head up. Both respond to the same changes, which is why the improvement can be quick once the loading changes.

What does not work is treating them as primary headaches. Analgesia taken most days for a load-related headache produces medication overuse on top of the original problem, and that is a harder situation than the one it started as.

Eyes, which belong on this page

An uncorrected or out-of-date prescription makes people lean toward the screen, and leaning toward the screen is the posture doing the damage. Bifocals are worse — they force chin elevation to read through the lower segment, which loads the upper cervical spine for hours.

An eye test is a legitimate part of managing screen-related neck pain and it is almost never suggested. If you have started leaning in, get the prescription checked before rearranging the desk.

The strengthening that actually matters

Deep neck flexor endurance and lower scapular strength, loaded progressively. Not stretching, which feels good and changes nothing durable, and not general fitness, which is worth doing for other reasons. Specific, progressive, and continued for months.

The reason is that the problem is capacity rather than position. A neck with adequate endurance tolerates a mediocre posture; a deconditioned one does not tolerate a good one.

Children and teenagers

Neck and upper back pain in adolescents who spend hours on devices is real and is increasing, and it is almost always load-related rather than structural. It responds to the same things — interruption, screen height, and building capacity — and it responds faster than it does in adults.

What it does not warrant is imaging. Scanning a teenager with load-related neck pain finds ordinary anatomy and introduces a structural explanation for a problem that does not have one, which is unhelpful for a person who has decades of neck ahead of them.

The one thing worth taking seriously

Neck pain that wakes somebody from sleep, comes with fever or weight loss, follows significant trauma, or is accompanied by any neurological change is a different conversation and belongs on the urgent page.

Everything else in this category is a loading problem, and loading problems are among the most fixable things we see.

What people ask about posture and screens

Am I damaging my spine?

There is no good evidence that phone use causes structural damage to the neck. What it does is load muscles continuously, and that produces real pain without producing damage. How work-pattern neck pain builds.

Do posture correctors help?

Briefly, as a reminder. They do not build the capacity that keeps the position without effort, and worn continuously they let the muscles do less rather than more. How facet pain is confirmed. A brace does the work the muscle needed to do.

Is my degenerative change from my job?

Degenerative change on cervical imaging is close to universal with age and appears in people with every kind of occupation. It is rarely attributable to one cause and it is frequently not the source of the pain. Trigger points and referred muscle pain.

What single change helps most?

Moving more often. Not the chair, not the desk, not the app. Interruption of sustained loading beats optimization of the position being sustained. How work-pattern neck pain builds. Interrupting the duration beats holding one correct position.
